Research Study

This week, we will discuss the research design and sample for your nursing clinical issue. The research design flows from the research question and outlines the plan for the study that will answer the research question. The design identifies the major components of the study. It is important to remember that there is no one best design for a research study.

  1. After you review the designs, describe which research design you would expect to find when searching for evidence relevant to your own research question from Week 2. Why? Explain your answer.

  1. The most common sampling method is the convenience sample; therefore, many of the studies that you find for evidence use this sampling method. What are the implications for using a convenience sample on the way you interpret and use the findings?

Identify two defense mechanisms used by Karen. Briefly refer to the characteristics of each defense mechanism you chose and explain how each defense mechanism is manifested in this case study.

Read the following case study:Karen, a 22 years old young woman, turned to treatment due to feelings of sadness and low self-esteem which she described to be feeling since the age of 15. At that time, she started experiencing difficulties with her academic performance at school, as well as loneliness and social difficulties. She also described difficulties in maintaining romantic relationships, as well as relationships with friends. When it comes to relationships with others, at first, she tends to easily create a meaningful and close connection, almost to the point of “giving her all”. After a short while, she is disappointed in one way or another, and tends to cut off any contact. In addition, she describes difficulties in understanding what she wants to do with her life, now that she finished her army service. She doesnt find any point or interest in anything and she is confused about her part in the adult world.
Karen is the youngest daughter of three sisters. Karens mom, lost her own mother while she was pregnant with Karen. Karens mom was very close to her mother, and experienced deep emotional distress, lack of strength and feelings of sadness and loss. These feelings lasted also after Karens birth. Karen described how her mother had once confided in her, that she had no energy at all after she gave birth to her. Not even to pick Karen up and calm her down when she used to cry as a baby. Moreover, it seems that Karens father was not able to be empathetic towards her mother, he was angry at her and distanced himself from her. This led to marital difficulties. Other than that, Karens development is described as overall normal. However, she did experience some difficulties in her adjustment to new educational settings.
Karen has been coming to treatment for three months now. She is consistent and doesnt miss any meetings. It’s clear that she values her therapist, as she keeps praising her, and the way therapy has been helping her. In the therapeutic meetings, she describes openly and thoroughly different social interactions as well as the relationships within her family. In the therapeutic relationship, it is evident that when the therapist says or behaves in a way that Karen does not like, she seems for a minute to be surprised and upset, but shortly after, explains why it was reasonable for the therapist to act as she did. When the therapist tries to cautiously relate to Karens initial reaction, Karen usually replies: I wasnt upset by it at all. However, the therapist noticed a pattern in which after such interactions occurred in the therapeutic meetings, Karen describes how other people make her feel angry and upset in the past few days.
Karen describes in therapy how much she loves and how attached she is to her mother. On the other hand, she is disengaged from her father, who she describes as a difficult and aggressive man. She is also very close to her oldest sister, she identifies with her and wishes to be like her. Karen describes her middle sister as being in the same group as her father, and Karen considers her to be as difficult as he is. Karen describes that throughout the years, when her parents used to fight, she used to support and defend her mom passionately. However, when the therapist refers to the role Karen took on herself in the familial relationships, Karen says its the role that every daughter should take. Analyze this case study using the following instructions:
Part A
1. Identify two defense mechanisms used by Karen. Briefly refer to the characteristics of each defense mechanism you chose and explain how each defense mechanism is manifested in this case study.
2. Define and explain the term transference and explain how it is manifested in this case. Part B
From the therapists point of view:
1. Try to sit in the therapists chair: choose a topic you would like to refer to empathetically (from Karens background or from the treatments description) and give an example of an empathetic sentence that could be said to Karen. Explain why you chose this topic and how it could contribute to Karen’s treatment.
2. Give an example of an interpretation that the therapist could give, and Karen would be able to benefit from. Explain why you chose this interpretation, what the goal of the interpretation is, and how it might contribute to Karen and her treatment.

A. Write a brief summary (suggested length of 2–3 pages) of the significance and background of a healthcare problem by doing the following:

1. Describe a healthcare problem

Note: A healthcare problem can be broad in nature or focused. 

2. Explain the significance of the problem.

3. Describe the current practice related to the problem.

4. Discuss how the problem impacts the organization and/or patient’s cultural background (i.e., values, health behavior, and preferences).

B. Complete the attached “PICO Table Template” by identifying all the elements of the PICO.

1. Develop the PICO question.

C. Describe the search strategy (suggested length of 1–2 pages) you used to conduct the literature review by doing the following:

1. Identify the keywords used for the search.

2. Describe the number and types of articles that were available for consideration.

a. Discuss two research evidence and two non-research evidence sources that were considered (levels I–V).

Note: Be sure to upload a copy of the full text of the aritcles with your submission

D. Complete the attached “Evidence Matrix” to list five research evidence sources (levels I–III) from scholarly journal sources you locate in major medical databases.

Note: Four different authors should be used for research evidence. Research evidence must not be more than five years old.

E. Explain a recommended practice change (suggested length of 1–3 pages) that addresses the PICO question within the framework of the evidence collected and used in the attached “Evidence Matrix.”

F. Describe a process for implementing the recommendation from part E (suggested length of 2–3 pages) in which you do the following:

1. Explain how you would involve three key stakeholders in the decision to implement the recommendation.

2. Describe the specific barriers you may encounter in applying evidence to practice changes in the nursing practice setting.

3. Identify two strategies that could be used to overcome the barriers discussed in F2.

4. Identify one indicator to measure the outcome related to the recommendation.

G. Acknowledge sources, using APA-formatted in-text citations and references, for content that is quoted, paraphrased, or summarized.


Articles have already been chosen, if can be helpful.  If you would like to choose an easier healthcare related problem or different articles, that is fine as well

Genitourinary and Adolescent Health and Management Plans

This week, there will be a variety of conditions assigned to you by your instructor pertaining to  genitourinary and adolescent health conditions (I chose Hemturia).  You are expected to present your initial topic including, but not limited to, the following items:

  • Physical exam findings

  • Differential diagnoses and rationale

  • Management plan to include diagnostic testing, medications if applicable, follow-up plans and referrals if needed

This week’s discussion will focus on the sick child and when it is necessary to refer. Please give an example from your clinical rotation of a child who was referred on to another health care provider. Include in your discussion the concept of “scope of practice” as defined in the location where you work. 


Follow-up after referral is a responsibility of the nurse practitioner and requires collaboration with other health care professionals as well as current knowledge of the problem being addressed. Not addressing follow-up can be a legal and malpractice issue.  (I chose referral to Pediatric ENT specialist due to multiple recurrent ear infections in the year).

If you had to help someone spot “fake news,” what would you tell them?  Choose a news article from a reputable media outlet and compare it to an online story that you know is fake.  Using the checklist provided on the factcheck web site, compare the two.  Although fake news is not new, it seems harder to distinguish and more accepted now.  Why?  How will this acceptance of fake news impact our democracy?

Discussion Question:

Johnson viewed health as efficient and effective functioning of the system, and as behavioral system balance and stability. Behavioral system balance and stability are demonstrated by observed behavior that is purposeful, orderly, and predictable. Consider a clinical-practice situation that demonstrates Johnson’s model of health. How can the nurse support this theory to return the patient to balance and stability?

Ann, a community nurse, made an afternoon home visit with Susan and her father. After the death of her mother, Susan had growing concerns about her father living alone. “I worry about my father all the time. He is becoming more forgetful and he has trouble seeing. Mom used to take care of him. I am not sleeping and I am irritable around him. Yesterday I shouted at him because he wouldn’t let me help him with his laundry. I felt terrible! I am at my wits’ end! My brothers and sisters do not want to put dad in a nursing home but they are not willing to help out. As usual, they have left me with all the responsibility. I work part time and have two small children to care for.” Susan’s father, Sam, sat quietly with tears filling his eyes. He was well nourished and well-groomed but would not make eye contact. Nurse Ann noticed that the house was clean and orderly. A tray in front of the TV had the remains of a ham sandwich and glass of ice tea. Mail was piled up, unopened on a small table near the front door. There was only one car in the driveway and the yard was in need of attention.

THEORY ASSIGNMENT

· You will find as you search middle range, and at times practice specific theory, that some of these theories overlap with what we have been referring to as nursing theory. An example of this is Orlando’s and Peplau’s theories. These theories are at times described as middle range and at other times as nursing theory. Thus. you will see there is no hard and fast rule as to labeling of level of theory but that theory flows from most abstract to least abstract.  The important take away for the next two weeks is that you develop an understand of the importance that both middle range theory and practice specific theory, is these two types of theory are less abstract and more easily applied to practice and research.

1. Compare and contrast middle-range theory and practice-specific theory. Discuss the impact the development of these theories has on nursing.

2. Locate a nursing research article (be sure it is a research article and not an opinion article) that clearly describes how a middle-range theory was used to guide or support the research. How did the author(s) apply the theory in their research?

3. Describe a practice scenario from your own nursing experience and identify a middle-range or practice-specific theory that could be used as a framework for nursing care delivery. Discuss how the theory could be used to support your nursing care of the patient. NOTE: Do not identify the patient’s name in compliance with HIPPA regulations.  Use of initials as patient identifiers is acceptable. 

For this assignment we will be discussing the wellbeing supportive physical environment. I have also uploaded my text book. Also refer to chapter 21 as well for additional reference. Please make sure all the requirements are met. I will do #3 the scenario from my own nursing experience
