For your final project, you will be researching an individual and their contribution to community and public health. Because of the size of this project, you will be doing the first part of this project in Week 3 then adding the final portion in Week 5. You have the option of writing a paper or putting together a presentation. Details are below the outline. Overall, you will be researching an historical figure in the public health world, their contribution to public health, and how their work helped shape the public health system. Furthermore, you will be analyzing how their contribution from the past continues to contribute to today’s public health system and how it might guide future work within the industry.

The special topic I chose to talk about is public health. Public health is a subject that I am really interested in. This article states that, “the Privacy Rule permits covered entities to disclose protected health information, without authorization, to public health authorities who are legally authorized to receive such reports for the purpose of preventing or controlling disease, injury, or disability” (OCR HIPAA Privacy, 2002).” The Rule helps with public health by allowing authorized persons to have access to protected health information and report any threats that could affect the public and individuals.

I chose public health because it is something I have always been interested in. It is important to me to help the public and individuals. As a whole, we must ensure that we are in great health and that we stay informed about any public health issues. I feel that public health should be important to everyone because it could easily affect you. Before I changed my major from nursing, I wanted to be a nurse in public health. But now with health care management I still would love to work where I can make a difference with the public. Preventative care is something I take serious. I feel everyone needs to be educated and take part in their own health care because that could dramatically help when it comes to the upkeeping of public health.

Describe the team that would deliver care and their individual roles.

Write a paper of 1,200-1,500 words that discusses the team-based approaches to the delivery of health services. The focus of the paper should be one type of health care environment, such as a hospital, primary care, or long-term care facility.

Within your paper, be sure to do the following:

  1. Present a patient case (this does not need to be detailed).
  2. Describe the team that would deliver care and their individual roles.
  3. Identify and describe the indicators and determinants of health that would influence or impact this case.
  4. Describe the impact this team-based approach would have on management and line staff.
